Club Sports Recognition Process

New Club Application


The UConn Club Sports program strives to provide leadership and athletic opportunities for UConn students. The Club Sports Program offers a competitive outlet for its participants, while enhancing their collegiate experience. Club Sports will give assistance, guidance, and resources for the clubs to operate successfully. This includes professional staff and a governing Club Sports Council.



  • To provide diversity in athletic competition at the University of Connecticut.
  • To provide assistance, guidance, and resources for the clubs to successfully operate; including professional staff and a governing club sports council.
  • To provide athletic opportunities that compliment students’ collegiate experience.



  • Every athlete on the official IMLeagues roster MUST complete and submit three required forms on in order to participate.
    • Participant Registration
    • Assumption of Risk, Permission to Treat and Confidentiality Agreement
    • Injury & Concussion Disclosure
  • Beginning Fall 2017, all Club Sports athletes will be required to fill out a Clearance to Play form, which can be found on the Student Health Services website:
    • Submit completed form by:
      • Dropping-off in-person at the Student Health Services Front Desk or at the Lock Box inside the front door
      • Scanning and uploading to the Student Health Portal
        • NOTE: When you are uploading the form to the portal, select “Health History Form” and upload the Clearance to Play


  • Freshmen do NOT have to fill out this form – Section 7 of the Student Health History Form IS the Clearance to Play. 


  • Participants must have the signature of their physician verifying they have had a physical examination within one year of the seasons start date.
